Preakness Healthcare Center is a skilled nursing facility with an approximate census of 280 residents. PHC admits residents for short-term post-acute care and long-term care. Will accept part-time applicants if full time not available.

Duties

• Conduct thorough assessments of patients' physical abilities and limitations.
• Develop and implement personalized treatment plans based on individual patient needs.
• Provide therapeutic exercises and interventions to improve strength, flexibility, and mobility.
• Educate patients and their families on treatment options, exercises, and home care strategies.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
• Document patient progress and modify treatment plans as necessary.
• Maintain compliance with all regulatory requirements and best practice guidelines.
• Work effectively with diverse populations, including geriatrics and those requiring acute care.

Requirements

• Valid state licensure as a Physical Therapist.
• Strong knowledge of physiology and anatomy.
• Experience in outpatient settings preferred; home health experience is a plus.
• Familiarity with developing care plans tailored to individual patient needs.
• Ability to work in various environments, including hospitals and home community care settings.
• Excellent communication skills for effective patient interaction and team collaboration.
• Compassionate approach towards patient care with a focus on improving quality of life.

Job Types: Full-time, Part-time, Contract

Pay: $59.00 - $60.00 per hour

Expected hours: 36 – 40 per week
